 Extracorporeal Shockwave Therapy (ESWT) is a non-invasive treatment that uses high-energy sound waves to stimulate tissue repair at a cellular level. It is one of the most effective interventions available for chronic tendon conditions and soft tissue injuries that have not responded to conventional physiotherapy alone — offering a clinically evidenced route to recovery without surgery or injections. 

At Mayfair Health, we use the Piezowave2 — a direct focused shockwave machine that delivers more precise, more effective treatment than the radial shockwave devices used in most clinics. Shockwave is always used as part of a comprehensive physiotherapy treatment plan, never as a standalone intervention.

Most clinics offering shockwave therapy use radial shockwave devices, in which the energy disperses outward from the probe in a broad, unfocused pattern. While effective for some superficial conditions, radial shockwave lacks the precision to reach deeper tissue structures accurately. 

The Piezowave2 uses piezoelectric crystals to generate sound waves that converge at a precise focal point — penetrating to an exact depth within the tissue. This direct focused approach means the therapeutic energy is delivered where it is needed, producing a stronger stimulus for tissue repair and making it effective for conditions where depth and precision are critical. It is the only truly direct focused shockwave technology available.

Before any shockwave treatment begins, your physiotherapist will carry out a thorough clinical assessment to confirm the diagnosis, establish whether shockwave is appropriate and explain exactly what to expect. Diagnostic ultrasound may be used to further assess the affected tissue. 

Each session lasts 15 to 20 minutes. A typical course involves three to six sessions spaced three to seven days apart, with most patients noticing improvement after the second or third session. The full benefit continues to develop over several weeks following the final session as tissue repair progresses.

What is shockwave therapy and how does it work?

ESWT uses high-energy sound waves to stimulate tissue repair at a cellular level — triggering increased blood flow, promoting new blood vessel formation, breaking down calcific deposits and stimulating collagen production. This accelerates the natural healing process in tendons and soft tissue that have become chronically painful or degenerated.

Most patients experience a toothache-like or pinching sensation, always set at your individual tolerance level. Some experience mild soreness for 24 to 48 hours post-treatment — a normal part of the healing response. 

Three to six sessions spaced three to seven days apart is typical. Your physiotherapist will advise based on your specific condition and review progress throughout. 

Shockwave is most effective for chronic tendinopathy — conditions where the tendon has become degeneratively thickened or calcified over time. It is generally not recommended for acute tendon injuries or in the presence of a complete tendon rupture. Your physiotherapist will confirm suitability at your assessment.
